Hours

Monday–Thursday, 8 a.m.–5:30 p.m.

Friday, 8 a.m.–4:30 p.m.

The fastest way to schedule an appointment is to contact Emily Waystack, coordinator for health and wellness, at emily.waystack@necmusic.edu or 617-585-1284.

You may also choose to contact TAK directly at 978-728-4957 or takcmhinfo@takmedgroup.com. TAK will respond to phone calls during normal business hours and will respond to email correspondences within 24 hours. If you are eligible, you will be sent an intake packet via Docusign to complete online prior to your appointment. Completing this packet will ensure that you are able to receive a prescription at the time of your first virtual appointment. All intake appointments are with Dr. Ramteen Rezai. Requests for another practitioner may be made for ongoing care after the initial appointment.

Psychiatric appointments are scheduled 3-4 weeks out. If you are in urgent need of medication prior to your appointment, Tufts Collegiate Center for Wellness is able to accommodate most refill requests until you become connected with TAK. You may contact Tufts by calling 617-636-6006 [Option 5]. If you have questions or need additional assistance, please contact Emily Waystack at emily.waystack@necmusic.edu or 617-585-1284.

Please be advised there may be a copayment or cost-share associated with this visit, which is determined by your health insurance provider. Unfortunately, TAK is unable to accept MassHealth at this time; however, they are able to make referrals to other providers who are accepting this insurance.
