Prospective Observational Study of Vocal Performance Majors
We are excited to collaborate with the Massachusetts Eye and Ear Infirmary. We would like to offer you the opportunity to participate in a study designed to collect information that will help us to better understand the factors that effect vocal function in singers. The study is not meant to diagnose or treat medical conditions in study participants. There are no treatments or medications associated with this study. You will, however, be notified if there are any clinical conditions requiring a medical evaluation (with a healthcare provider of your choosing) after your exam has been completed. New study participants will be asked to sign consent and HIPAA forms and fill out various questionnaires. In addition, you will be asked to undergo a laryngeal examination. We ask that you perform these activities as follows: on a yearly basis; upon completion or withdrawal from the program; and when you have a problem with your voice. - Laryngeal and Videostroboscopic exams (exam of your throat and vocal folds and
video of your vocal folds in motion) - Acoustic and aerodynamic measurements while you speak and sing
Returning Study Participants will complete (to be completed annually and upon completion/withdrawal from program)

Vocal Difficulty
If episodes of vocal difficulty occur, please contact Leah McKinnon-Howe. She will refer you to a provider of your choice as involvement in the study does not constitute medical care. If the episode of vocal difficulty requires medical referral you will be asked to complete the questionnaires (below). This is not considered medical care.

Testing Dates and Time:
Saturday, September 29 or Sunday, September 30, 2012
8:30 a.m. – 12:30 p.m.
Please plan to arrive promptly at 8:30 a.m. There will be several clinicians performing the testing. The total testing time should take anywhere from 45– 60 minutes.
Transportation by MBTA: Take the Red Line to Charles/MGH Station (handicap accessible). Exit the platform, walk through the turnstile, and turn right. Proceed straight ahead. The Liberty Hotel will be right in front of you. When facing the hotel, walk to the left. The Massachusetts Eye and Ear Infirmary is located just beyond the hotel at the corner of Charles Street and Fruit Street. The hospital is a big tower and there is one entrance. Take the elevator the 11th floor. Please e-mail Leah McKinnon-Howe MS, ANP- BC to confirm your preference of testing date.
