Use this link for the 2013-2014 Health Report for your review and to download. The confidential information that you provide will allow us to assist you with your health care needs. The Commonwealth of Massachusetts requires documentation of certain immunizations, or proof of immunity, to certain illness (see page 3 of the health report) for all full-time students enrolled in Massachusetts, regardless of age or degree status. These immunizations help to insure a healthy campus. Documentations of your immunization status (on official letterhead) can be obtained from your health care provider, high school, previous undergraduate program, or through military service, and attached to the health report.
You must submit your completed health report to NEC Health and Counseling Services by January 13, 2014. Incomplete or missing health reports will result in a $150 fine, a delay in registration, and a fee for immunizations administered at the Health Center. We are implementing a new vaccine management system that requires students to enter the vaccine information found on their health report into a computer based portal. We will be emailing you information about how and where to enter the vaccine information once the link is created. (Please make a copy of your completed health report so before sending to us so you can reference the dates of your immunizations when entering the information.) The accuracy of your electronic submission will be verified through our review of your completed health report.
Taking a proactive approach to your health care helps to insure effective and comprehensive management of health problems before they arise. You would benefit from establishing a relationship with a member of our clinical staff if you have any ongoing medical issue or take prescription medications on a regular basis. Please call us (617-585-1284) at your earliest convenience to schedule an appointment. Best wishes for a successful year and we look forward to meeting you.
